Who will benefit from these services?
* Family members who live in a different city and are unable to evaluate the changing physical, mental, and soical needs of a loved one and obtain the necessary services in order to maintain an adequate level of functioning in the community.
* Family members who do not have the experience and /or time to seek out available services in the community and monitor their loved one's living situation.
* The Older Adult who is struggling with lifestyle, mental and physical health changes. Information, various forms of support, and options for care can be made readily available.
